大学住房合理住宿政策

节我。       简介及背景

太阳城娱乐官网大学科尔尼分校（“太阳城娱乐官网”或“大学”）认识到在必要时为残疾人使用和享受大学住房的住房政策和做法提供合理住宿的重要性。 本政策解释了申请合理住宿的具体要求和指导方针。  太阳城娱乐官网保留根据情况需要随时修改本政策的权利。

第二部分。      要求合理住宿的程序 （不包括根据《美国残疾人法案修正案》对服务性动物的请求）

学生残疾服务办公室（“残疾服务”）负责评估是否批准或拒绝在大学住房中提供合理住宿的请求。 在评估请求时，残疾服务部将根据需要与住宿生活办公室（“住宿生活”），太阳城娱乐官网 ADA/Section 504协调员以及太阳城娱乐官网咨询和护理办公室协商，以确定所请求的住宿是否必要和合理。 居住或打算居住在大学宿舍的残疾人士，如果认为自己需要合理的住宿，必须联系残疾服务中心。

在大学住宿政策和实践中，合理住宿的要求受以下要求的约束：

  1. 申请住宿

  •  An individual with a disability must complete the “残疾人士住宿申请表格” (the “Request Form”) to request a reasonable accommodation.    申请表格副本可向残疾人士服务处索取。  如果个人因残疾而需要协助填写申请表，残疾服务部将协助填写申请表。

  •  太阳城娱乐官网将在任何时候接受并考虑在大学住房合理住宿的请求。  提出住宿要求的个人应在搬进大学宿舍之前尽快填写并提供残疾服务申请表。 However, if the request for accommodation is made fewer than 60 days before the individual intends to move into University housing, 太阳城娱乐官网 cannot guarantee that it will be able to meet the individual’s accommodation needs during the first semester or term of occupancy.

 2. Information that May Be Requested for 住房-Related Reasonable 住宿要求

  • 不明显的残疾/必要性

    •    If the disability is obvious but the need for the accommodation is not obvious, the University may require the individual to complete the 大学宿舍合理住宿证明表格 (“Verification Form”) and designate a reliable third party who can verify that the requested accommodation is necessary to provide the individual an equal opportunity to use and enjoy University housing, but may not seek information about the individual’s disability.

    • If the disability and necessity for the accommodation are not obvious, Disability Services will require the individual to complete the Verification Form and designate a reliable third party who can verify that  the individual has a disability and that the requested accommodation is necessary to provide the individual an equal opportunity to use and enjoy university housing.

    • A reliable third party is someone who is familiar with the individual's disability and the necessity for  the  requested accommodation. 可靠的第三方包括但不限于医生或其他医疗专业人员、同伴支持小组或非医疗服务机构（例如，全国聋人协会）。

    • If the third party returns the Verification Form without sufficient information for Disability Services to determine whether an accommodation is necessary, the Director of Disability Services will inform the individual in writing of the verification’s insufficiency and may request additional information, including speaking directly with the individual supplying the third-party verification, within seven (7) business days of receiving the verification.

3. 确定合理性

4. 住宿批准

    • 如果残疾服务部门确定所要求的住宿是必要的，并且不是不合理的，它将在其确定后的七(7)个工作日内以书面形式联系个人，安排会议讨论住宿的实施情况。

5. 拒绝住宿/上诉

    • If Disability Services determines a requested accommodation is necessary but unreasonable, Disability Services will contact the individual, in writing, within seven (7) business days of its determination and engage in an interactive process with the individual to determine if there are alternative accommodations that might effectively meet the individual’s disability-related needs.

    • 所有申诉均由太阳城娱乐官网大学ADA/第504条合规官审查。 If the appeal is denied, the University of Nebraska’s ADA/Section 504 Compliance Officer shall provide written notification of the denial to the individual and a written explanation with all of the reasons for the denial.

保密和记录保存

7. Non-retaliation条款
